You are the one place people look to for info. Could start as just a content business to begin, then could branch out to offering services.
I think one major obstacle is people don't know about it. How do you reach someone who wants or needs a procedure but wants to pay less for it? In that respect, a niche could be easier to start with and then expand from there.
Yeah, the one thing I have the most conviction on right now is that there is no "medical tourism guy" or media company. Content is currently the only path I see to a scalable global brand, because otherwise it is too sparse.
Even then, there's probably a case to be made for starting with a niche medical category.
Another approach would be starting a niche audience, such as nomadic techies and founders from America. Doing this might make it easier to build a community with enough overlap to create value for one another. Second, there's a case for just being the primary care provider for this group of people, which is a stable provider and source of truth across different locations and treatment categories.
